Care First home health provides Home and Community Habilitation. Home and Community Habilitation is a direct service that consists of services designed to assist individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to reside successfully in home and community-based settings.

PFDS Waiver:
The Pennsylvania Person/Family Directed Support Waiver is designed to help individuals with an intellectual disability, autism, or developmental disability to live more independently in their homes and communities and to provide a variety of services that promote community living, including self-directed service models and traditional, agency-based service models.
Consolidated Waiver:
The Pennsylvania Consolidated Waiver is designed to help individuals with an intellectual disability, autism or developmental disability to live more independently in their homes and communities and to provide a variety of services that promote community living, including self-directed service models and traditional, agency-based service models.
This is direct service provided by our experienced and compassionate caregivers in the homes of our consumers, other residential settings and community. This service is geared towards assisting individuals in acquiring, retaining, and improving self-help, socialization and adaptive skills. We help you or your loved one by improving quality of life and enhancing independence.
This service can be provided in the private home of the consumer, home of a family member, friend or any other home acceptable to the individual/family.
